Appointments
If you are unable to keep an appointment for any reason, we ask that you call immediately to reschedule the appointment within 7 days of cancellation if possible.
Payment Policies
If we are not billing insurance for you, all charges are due at the end of each visit. Payment options are available upon request.
Insurance and Billing
Optima Health & Vitality Center will call to verify your insurance benefits and notify you of your insurance coverage. Charges will be submitted to your insurance company by our office on your behalf. All deductibles, co-pays and co-insurances are due at the time services are rendered.
